Insurance Information

Here at The Teggatz Clinic, we do NOT bill insurance.

Even though we do not bill your insurance provider for you – there are a few steps you can do to see if you could be refunded some money from your insurance.

  1. Call the 1-800 number on the back of your insurance card.
  2. Ask if you have “Out of Network” Benefits:-If you DO NOT have out of network benefits – do you have an HSA or similar account that you could use?
    • If you DO HAVE out of network benefits
    • ask what your specific benefits are.

**Typically: once you’ve met your deductible, then they will pay a percentage.

**Ask your insurance company for the process on “self-submitting a claim for reimbursement.”

If you have out of network benefits – after you have paid for your services, we will provide you with:

  1. Receipt
  2. HCFA Form (insurance form that has all the codes and numbers insurance would ask for)

With these two forms – you submit them to your insurance for reimbursement based on your specific out of network benefits plan.

FYI: Insurance plans do NOT reimburse for packages but for single item purchases only.
